Why Is This Medication Prescribed

Pramipexole is used alone or with other medications to treat the symptoms of Parkinson’s disease , including shaking of parts of the body, stiffness, slowed movements, and problems with balance. Pramipexole is also used to treat restless legs syndrome . Pramipexole is in a class of medications called dopamine agonists. It works by acting in place of dopamine, a natural substance in the brain that is needed to control movement.

Johns Hopkins researchers believe they may have discovered an explanation for the sleepless nights associated with restless legs syndrome , a symptom that persists even when the disruptive, overwhelming nocturnal urge to move the legs is treated successfully with medication.

Neurologists have long believed RLS is related to a dysfunction in the way the brain uses the neurotransmitter dopamine, a chemical used by brain cells to communicate and produce smooth, purposeful muscle activity and movement. Disruption of these neurochemical signals, characteristic of Parkinsons disease, frequently results in involuntary movements. Drugs that increase dopamine levels are mainstay treatments for RLS, but studies have shown they dont significantly improve sleep. An estimated 5 percent of the U.S. population has RLS.

The small new study, headed by Richard P. Allen, Ph.D., an associate professor of neurology at the Johns Hopkins University School of Medicine, used MRI to image the brain and found glutamate a neurotransmitter involved in arousal in abnormally high levels in people with RLS. The more glutamate the researchers found in the brains of those with RLS, the worse their sleep.

Allen says there are already drugs on the market, such as the anticonvulsive gabapentin enacarbil, that can reduce glutamate levels in the brain, but they have not been given as a first-line treatment for RLS patients.

How Is Restless Legs Syndrome Diagnosed

Since there is no specific test for RLS, the condition is diagnosed by a doctors evaluation. The five basic criteria for clinically diagnosing the disorder are:

  • A strong and often overwhelming need or urge to move the legs that is often associated with abnormal, unpleasant, or uncomfortable sensations.
  • The urge to move the legs starts or get worse during rest or inactivity.

What Treatment Options Are Available

The first and often best option is massage. Massaging the legs every evening helps prevent symptoms most of the time. Massage before sleep helps. I recommend it as a first-line treatment before considering medications. Warm compresses or cold compresses may help. My patients who use electric massages get great benefits.

The next step is to swap out medications that can worsen symptoms such as certain antidepressants and antihistamines. If your doctor finds that you have low levels of iron, replacing it can help, too. The last resort is using medications made to treat restless legs, and the good news is that there has been progress in finding new medications.

What Causes Restless Leg Syndrome

The cause of restless leg syndrome isnt known for sure, and the source of the condition may vary between people who have it. Theres some evidence it may be caused by genetic factors while certain lifestyle factors such as iron deficiency, alcohol, caffeine and nicotine seem to aggravate it. It can also be a symptom of another condition like neuropathy or end-stage renal disease. RLS also often occurs with or starts before the onset of a mental health condition.

However, a body of evidence suggests a connection between restless legs and issues in the basal ganglia area of the brain, which relies on the chemical dopamine to help regulate your movements. A loss of dopamine is the main cause of Parkinsons disease, a progressive disorder that most noticeably causes tremors, gait changes and other movement-related symptoms.

Rls Leading To Anxiety

Itâs also possible that restless legs cause anxiety, rather than the other way around.

Support for this theory is based on the idea that RLS leads to poor sleep quality, and poor sleep quality is a known contributor to stress and anxiety.

The belief is not that RLS itself necessarily causes anxiety . Rather, that the main symptoms of RLS constant uncomfortable feelings when the person is getting ready to rest that force them up and about, as well as the inability to get a good nightâs rest may contribute to overall anxiousness that could get worse as the disorder continues.

Antidepressants arent the only medication that can cause RLS. Antipsychotic medications, which are designed to block dopamine, also increase the likelihood youll develop restless leg syndrome, as well as other movement-related side effects called extrapyramidal side effects.
