Restless Legs Syndrome: An Updated Algorithm

Not all patients with RLS need treatment. Patients with mild disease may not need drug therapy or other interventions, whereas those with severe symptoms may require multiple medical treatments. Similar to the approach for patients with back pain, therapeutic decisions should be based on the frequency and severity of symptoms and the disability that results from them. Several algorithms for guiding physicians on how to treat RLS have been developed and they all have fairly similar suggestions, which vary, mostly due to the availability and government approval of medications in the authors countries. Recommendations and suggestions for treating RLS in this article are based on 1 of the oldest of these algorithms , but they have been updated by currently available medication, interval medical literature, and the authors clinical experience.

Other Restless Legs Triggers

Other health conditions can also cause restless legs. This is whats called secondary restless legs syndrome:

  • Pregnancy. According to Dr. Avidan, about 20 percent of pregnant women report restless legs symptoms. This is more likely in the last three months of pregnancy, and iron deficiency is usually the culprit. If symptoms crop up when youre expecting, your doctor will probably test the level of iron in your blood and prescribe iron supplements if its too low. The good news is that your restless legs symptoms will probably go away soon after you have your baby.
  • Medical conditions. Restless legs symptoms are also seen in some people with kidney failure and diabetes. Treating these conditions will often calm the restless legs.

The Restless Legs Syndrome Foundation suggests using a sleep diary to pinpoint your personal triggers and gauge the severity of your symptoms. Always tell your doctor if your symptoms get noticeably worse.

Restless Leg Syndrome Can Occur In Only One Leg And There Are A Few Risk Factors That Can Make This Happen

Restless leg syndrome is a condition where a person experiences pain, tingling, a crawling sensation or uneasy feeling in their legs, says J. Mark Anderson, MD, DABFM, of Executive Medicine of Texas and who is board certified in family medicine.

This condition is often worse at night. Most cases involve both legs, but it can happen in just one leg.

Single leg restlessness is most common in people who stand for long periods of time during the day, shifting the majority of weight to one side, and in pregnant women who are in their last trimester.

And, like RLS that affects both legs, sometimes the triggering agent for single leg RLS is not known.

Dr. Anderson adds, Getting up from bed and walking around, soaking in the tub, and massage have all been identified as ways to reduce the symptoms of RLS.

And you may also want to just roll with the punches, as the saying goes. In other words, if youre lying in bed, and you just cant keep your legs still, then go ahead, move them while remaining in bed.

Do kicks, knee extensions, whatever it takes to relieve the urge to keep moving your legs.

I speak from experience. Ive had a few mysterious episodes of RLS. This includes while I was sitting at my computer. I found that what worked best was giving in.

While I worked at the computer, I kept my legs active. My attitude was, The heck with this if my legs want to move, then for Petes sake, just move them! Plus, this burns more calories than if you sit like a bump on a log.

What Is Restless Legs Syndrome


Restless legs syndrome , also called Willis-Ekbom Disease, causes unpleasant or uncomfortable sensations in the legs and an irresistible urge to move them. Symptoms commonly occur in the late afternoon or evening hours, and are often most severe at night when a person is resting, such as sitting or lying in bed. They also may occur when someone is inactive and sitting for extended periods . Since symptoms can increase in severity during the night, it could become difficult to fall asleep or return to sleep after waking up. Moving the legs or walking typically relieves the discomfort but the sensations often recur once the movement stops. RLS is classified as a sleep disorder since the symptoms are triggered by resting and attempting to sleep, and as a movement disorder, since people are forced to move their legs in order to relieve symptoms. It is, however, best characterized as a neurological sensory disorder with symptoms that are produced from within the brain itself.

RLS is one of several disorders that can cause exhaustion and daytime sleepiness, which can strongly affect mood, concentration, job and school performance, and personal relationships. Many people with RLS report they are often unable to concentrate, have impaired memory, or fail to accomplish daily tasks. Untreated moderate to severe RLS can lead to about a 20 percent decrease in work productivity and can contribute to depression and anxiety. It also can make traveling difficult.

How Is Restless Legs Syndrome Diagnosed

Since there is no specific test for RLS, the condition is diagnosed by a doctors evaluation. The five basic criteria for clinically diagnosing the disorder are:

  • A strong and often overwhelming need or urge to move the legs that is often associated with abnormal, unpleasant, or uncomfortable sensations.
  • The urge to move the legs starts or get worse during rest or inactivity.
  • The urge to move the legs is at least temporarily and partially or totally relieved by movements.
  • The urge to move the legs starts or is aggravated in the evening or night.
  • The above four features are not due to any other medical or behavioral condition.

Diagnosing RLS in children may be especially difficult, since it may be hard for children to describe what they are experiencing, when and how often the symptoms occur, and how long symptoms last. Pediatric RLS can sometimes be misdiagnosed as “growing pains” or attention deficit disorder.

    What Does It Feel Like

    What Is The Prognosis For People With Restless Legs Syndrome

    RLS is generally a lifelong condition for which there is no cure. However, current therapies can control the disorder, minimize symptoms, and increase periods of restful sleep. Symptoms may gradually worsen with age, although the decline may be somewhat faster for individuals who also suffer from an associated medical condition. A diagnosis of RLS does not indicate the onset of another neurological disease, such as Parkinsons disease. In addition, some individuals have remissionsperiods in which symptoms decrease or disappear for days, weeks, months, or yearsalthough symptoms often eventually reappear.     What Causes Restless Legs Syndrome

    In most cases, the cause of RLS is unknown . However, RLS has a genetic component and can be found in families where the onset of symptoms is before age 40. Specific gene variants have been associated with RLS. Evidence indicates that low levels of iron in the brain also may be responsible for RLS.

    Considerable evidence also suggests that RLS is related to a dysfunction in one of the sections of the brain that control movement that use the brain chemical dopamine. Dopamine is needed to produce smooth, purposeful muscle activity and movement. Disruption of these pathways frequently results in involuntary movements. Individuals with Parkinsons disease, another disorder of the basal ganglias dopamine pathways, have increased chance of developing RLS.

    RLS also appears to be related to or accompany the following factors or underlying conditions:

    • end-stage renal disease and hemodialysis
    • iron deficiency
    • certain medications that may aggravate RLS symptoms, such as antinausea drugs , antipsychotic drugs , antidepressants that increase serotonin , and some cold and allergy medications that contain older antihistamines
    • use of alcohol, nicotine, and caffeine
    • pregnancy, especially in the last trimester in most cases, symptoms usually disappear within 4 weeks after delivery
    • neuropathy .

    Sleep deprivation and other sleep conditions like sleep apnea also may aggravate or trigger symptoms in some people. Reducing or completely eliminating these factors may relieve symptoms.

    What Research Is Being Done

    The mission of the National Institute of Neurological Disorders and Stroke is to seek fundamental knowledge about the brain and nervous system and to use that knowledge to reduce the burden of neurological disease.     Quetiapine And Restless Legs


    I’ve recently been prescribed Quetiapine 150mgs and find about an hour after taking it I get terrible restless legs for a couple of hours. I have started taking it earlier so I can get to sleep at a reasonable hour as I have to stay up until the ‘twitchy’ feeling in my legs has passed. Lying in bed is unbearable. Has anyone else had this side effect with Quetiapine and if so, did it pass? I started on 25mgs doubling up each night until I reached 150mgs. I have been on it for 7 nights now. Any feedback would be appreciated. Thanks in advance

      I did get restless legs initially when I started to take it – the instant release. I am on extended release now and do not experience that side effect. Whether it was the change in preparation or whether it wore off I am not sure.

    Hope your not too bad right now!!

    I’ve been on Q since 2008… And experienced the same symptoms as you…but as the years progressed the symptoms got worse… Thru the day I was trying to stretch my muscles every 5 mins or so, now at 35 I look like a complete idiot at pain relief classes as I’m pulling my body into all sorts of postions to relieve the pain… Although I do notice some release in positions that are not in the normal range…. But apparently some physios say this is coz I was born double jointed!! I am totally not convinced!

    Anyone elses experiences would be greatly appreciated…, love you all x

  • Lifestyle Strategies May Help

    For a lot of people with mild to moderate RLS symptoms, a number of lifestyle or habit changes can make a big difference. Regular moderate exercise can reduce symptoms in some people, though over-exercising, to the point of pain or aching in joints and muscles, can exacerbate RLS.

    Planning ahead works for many people. Try doing active chores or going for a walk during the evening if thats when symptoms usually occur. Or engage in a mentally distracting activity such as putting together a crossword puzzle, knitting, or talking with a friend, suggests the American Thoracic Society.

    Also useful: Consider reducing your consumption of alcohol and caffeine, which can set off symptoms.

    And pay attention to the amount of sleep you’re getting RLS often interferes with slumber, and insufficient sleep itself can trigger restless legs symptoms, Bertisch says.

    To compensate, people may turn to over-the-counter sleep medication, usually an antihistaminebut thats ill-advised. Antihistamines are actually thought to make RLS worse, Bertisch says. Instead, try other techniques for getting a good nights sleep.

    Certain other medications, including several types of antidepressants, can worsen RLS as well. Talk with your doctor about other options for easing depression.
