Sudden cardiac deaths account for nearly half a million deaths per year, but their underlying risk factors remain unresolved.

A growing body of evidence suggests that sleep apnea, where a personâs breathing pauses or becomes shallow while asleep, increases a personâs chances for developing heart disease.

This same group of Mayo researchers found people with sleep apnea had 2.6-fold higher risk of SCA while sleeping during the night between 10 p.m. and 6 a.m. In the general population, these hours are typically connected with the lowest number of sudden cardiac deaths.

Abstract

1. Introduction

Menieres disease is a condition characterised by fluctuating and progressive hearing loss, aural fullness, tinnitus, and intermittent attacks of vertigo. Although the aetiology of MD is largely unknown, the pathology is the result of hydropic distension of the endolymphatic system . Various factors such as stress are known to trigger or contribute to MD .

Obstructive sleep apnoea syndrome is a sleep-related breathing disorder where episodes of apnoea/hypopnea occur during sleep due to a collapse of the upper airway. The direct consequences of the collapse are intermittent hypoxia and hypercapnia, recurrent arousals, and increase in respiratory efforts, leading to secondary sympathetic activation, oxidative stress, and systemic inflammation . The cardiovascular complications of OSA are well documented , but the neurotological consequences have only been studied recently . A mainstay of treatment for OSA is continuous positive airway pressure , which corrects the hypoxia and associated body stresses.

We report a case to illustrate the relationship between MD and OSA and highlight some of the challenges encountered. The relevant literature will be reviewed and discussed from the perspective of the otologist/neurotologist.

    Sleep apnea is a disorder characterized by a reduction or pause of breathing during sleep. It is common among adults, becoming more common in children. Although a diagnosis of sleep apnea often will be suspected on the basis of a person’s medical history, there are several tests that can be used to confirm the diagnosis. The treatment of sleep apnea can be either surgical or nonsurgical.

    An apnea is a period of time during which breathing stops or is markedly reduced. In simplified terms, an apnea occurs when a person stops breathing for 10 seconds or more. If a person stops breathing completely or take less than 10% of a normal breath for a period that lasts 10 seconds or more, this is an apnea. This definition includes complete stoppage of airflow. Other definitions of apnea that may be used include at least a 4% drop in oxygen in the blood, a direct result of the reduction in the transfer of oxygen into the blood when breathing stops.

    Another index that is used to measure sleep apnea is the respiratory disturbance index . The respiratory disturbance index is similar to the apnea-hypopnea index; however, it also includes respiratory events that do not technically meet the definitions of apneas or hypopneas, but do disrupt sleep.

    A few days after my 3rd son Brennan was born, I suffered from classic benign positional vertigo. Looking back on the course of events, I had a eureka moment last night that literally kept me up in bed.

    Benign Positional Vertigo is a well-described inner ear condition that otolaryngologists like myself treat all the time. The classic description is when you feel dizzy, like the room is spinning, just after a sudden head movement, either up or down, or side to side. The spinning will usually last a few seconds, and you may have residual nausea and imbalance for hours to days. Its typically preceded by an infection, head trauma, stress, or in many cases, no significant events at all .

    The Dix-Hallpike maneuver is performed to make the diagnosis and the Modified Epley is then continued on to cure the problem if the Dix-Hallpike is positive. In my experience, the Epley maneuver works about 80-90% of the time to cure the problem instantly if the Dix-Hallpike is strongly positive. Its one of the more gratifying maneuvers/procedures that I perform.

    Any infection, whether a common cold or sinusitis, causes swelling in the nose and throat which narrows the upper airway, which narrows the throat even further, leading to more obstructions, causing more reflux, leading to more throat inflammation and narrowing.

    Sleep Apnea Headaches Typically:

    • Are felt on both sides of the head
    • Dissipate patient wakes up and breathes normally .
    • Regularly reoccur
    • Are only felt on one side of the head
    • May last for hours or days
    • Can be accompanied by visual impairments
    • Include nausea and vomiting
    • Include light sensitivity

    The different types of headaches that can be caused by sleep apnea include:

    • Cluster headaches – Headaches that occur in clusters or patterns during cluter periods that may last for weeks or months.
    • Hypnic headaches – Moderate throbbing headaches that wake the sufferer up one or more times during the night.
    • OSA headaches -Headaches caused by the widening of blood vessels.

    Sleep apnea and headaches go hand in hand, especially in older patients who often wake up a few hours after sleeping with a headache. Generally headaches due to sleep apnea occur between 4 AM – 9 AM, but they can also occur at night before bed.

    Unfortunately, once you begin to experience sleep apnea headaches you can get caught in a cycle with pain on a daily basis. This is because chronically losing sleep creates a debt in your system that leads to stress and inflammatory changes, especially when hypoxemia, an abnormally low concentration of oxygen in the blood occurs.

    Those with chronic headaches may be encouraged to keep a journal for three months to track triggers and patterns for attacks. However, you may also be advised to be tested for sleep apnea with a simple, at-home test.

    Yes, these diseases are associated with vertigo. But they arent the real underlying cause.

    In fact, doctors have no idea what causes these diseases. They say Ménières causes vertigo and then state that vertigo is the main symptom of Ménières.
