Inspire Upper Airway Stimulation

How it works: Inspire therapy is an implanted system that senses breathing patterns and delivers mild stimulation to key airway muscles keeping the airway open during sleep.

When to try it: After CPAP refusal or failure.

Appropriate AHI: 15 to 65.

Other considerations: In addition to the AHI consideration, patients might be candidates for Inspire therapy if they are unable to use or get consistent benefit from CPAP, are not significantly overweight, and are over the age of 22. What Are The Benefits Of Inspire For Sleep Apnea

As weve mentioned, the primary treatment for sleep apnea is continuous positive airway pressure therapy .

This is a highly effective treatment that uses a machine attached to a hose and a face mask which the patient wears during sleep. The device sends air through the mask to the patient, which continuously keeps the airways open to prevent sleep apnea.

The problem with CPAP therapy is many patients find the treatment invasive and disturbing to their sleep. Furthermore, patients who rely on CPAP treatment need to take the machine with them on overnight trips.

Inspire for sleep apnea solves all of these problems because the device is implanted into the patient. Patients only need to take the small remote controller with them when going on holiday or sleeping away from home, so it is far more convenient than CPAP treatment.

After surgery, patients return home to begin recovery.

Siesta Medicals Airlift Hyoid Suspension Study Shows 74% Reduction In Ahi

Siesta Medical, Inc. announced the online publication of a recent study of the AIRLIFT procedure titled Hyoid Suspension With UPPP for the Treatment of Obstructive Sleep Apnea in the Ear, Nose & Throat Journal.

This multicenter study evaluated the safety and efficacy of AIRLIFT hyoid suspension when combined with procedures for the palate on 39 patients treated at 3 centers. The median apnea-hypopnea index reduction was 74% while the AHI was reduced from 49.9 preoperatively to 15.4 after treatment. All patients saw a positive drop in their AHI assessment and 84% of patients with severe sleep apnea, the riskiest level of sleep apnea, improved to mild or moderate sleep apnea. In addition, the efficacy of the procedure appeared to be independent of BMI, even in very high BMI patients.

Continued Excessive Daytime Sleepiness

Sometimes individuals with treated sleep apnea still experience excessive daytime sleepiness , a chronic condition, and need help with improving wakefulness. New Sleep Apnea Surgeries

Recently, doctors have started promoting more minimally invasive surgical techniques to help people overcome sleep apnea. One of these that has been around for a few years, is called AIRLIFT. Practitioners describe this as a facelift for your throat. The goal is to suspend your airway further forward to help expand your airway so that you can breathe easier. Surgeons do this by putting stitches around your hyoid bone, holding it forward. This should create extra space in the airway, reducing or eliminating sleep apnea.

However, actual studies show it doesnt actually eliminate sleep apnea. In one study, promoted by AIRLIFT, the surgery reduces a persons AHI from 40 to 22. This is a reduction from severe to moderate sleep apnea, but moderate sleep apnea is still dangerous and needs treatment. People need to reduce their AHI below 15 to significantly reduce their risk, although even lower is better.

Another surgical technique being promoted is a less invasive form of traditional surgery reducing the volume of mouth and throat structures to expand the airway. Preliminary trial results for this surgery were published just last week. They show patients saw their AHI reduce from about 50 to 20, again from severe to moderate. In addition, there were two serious complications among the surgery patients.

Neither of these treatments can eliminate the need for further sleep apnea treatment.

How Does Inspire For Sleep Apnea Work

Inspire treatment for sleep apnea begins with a comprehensive evaluation, including a procedure called Drug Induced Sleep Endoscopy , that involves examining the upper airway while under general anesthesia, and it is aimed at establishing the areas of obstruction in the upper airway and determining candidacy for the Inspire procedure.

In candidates that are appropriate and decide to proceed with Inspire, the surgical procedure will involve inserting the Inspire device just under your clavicle, similar to a cardiac pacemaker.

The device is composed of three elements: the generator, the breathing sensor lead, and the stimulation lead.

While you sleep, the breathing sensor lead monitors your breath rate. If there is a cessation in your breathing, the generator sends a pulse of energy through the stimulation lead, which is attached to the hypoglossal nerve. The hypoglossal nerve is responsible for moving the tongue and other muscles of the airway.

When the stimulation lead of the Inspire device triggers the hypoglossal nerve, your airway is held open by your own tongue and throat muscles, which prevents it from collapsing and causing obstruction.

The device is turned on and off with a small remote controller patients turn the Inspire device on as they go to bed and turn it off when they wake up. The device doesnt require any changes in behavior other than turning the device on and off, which is one reason it is such a popular treatment for sleep apnea.

New Sleep Apnea Treatment Offers Cpap Alternative

People with obstructive sleep apnea who havent found relief may benefit from a new pacemaker-like device that keeps the airway open during sleep. Potential Difficulties When Using Cpap Equipment

Although CPAP equipment is the most effective treatment for OSA, there can be an adjustment period and some people experience difficulties when they first start using them. Potential difficulties include:

  • Dry nose and sore throat
  • Congestion and runny nose
  • Irritation of eyes and skin on the face
  • Leaking around the mask
  • Mild discomfort in the mornings
