Why Does Vertical Opening Matter

A Simple Fix For Snoring And Sleep Apnea

The lower jaw rotates backwards as it opens which further narrows your airway and makes snoring worse .

If your jaw is protruded with a mandibular advancement device and your jaw falls open your airway may be narrower than without using an MAD.

See the three X-ray diagrams below, to understand why vertical opening with an MAD should be avoided.

The red dots highlight how the airway increases / decreases with protrusion and vertical opening.

Images courtesy of Dr Ulrich Huebers DDS.

The diagram shows the size of the airway in your throat without a mandibular advancement device .

The diagram shows the ideal situation. The mandibular advancement device advances the jaw and opens the airway in your throat.

The third diagram shows how the airway becomes narrower with both protrusion and vertical opening. Disaster.

Minimal vertical opening should routinely be considered ideal. More than this may be necessary in selected cases where:

  • the tongue is large
  • the jaw is small

However, opening beyond 5mm is uncomfortable, inhibits lip closure and detrimentally impacts long-term use. A design principle for MADs is control of vertical opening .

Finally, consider your sleep position. Avoid sleeping on your back and ensure that your neck is inline with your body. Choose a supportive pillow. Learn more about positional therapy and anti-snore pillows.

Breathe Right Nasal Strips

These clear plastic adhesive strips are worn over the bridge of the nose and have spring-like bands that lift the nasal passages, supposedly opening them wider to improve airflow.

Each strip is individually packaged and has a peel-away backing for easy application over the nose.

However, because these disposable strips are only good for one use each, theres quite a bit of unnecessary waste associated with this product.

Though the Anti Snore Chin Strap is advertised on its packaging as being made from a breathable and comfortable material, John found the device made his head feel a little too warm in the spots it covered about 15 minutes after he put it on.

He described it as, not the worst thing, but definitely not comfortable either.

Shortly after John fell asleep that night, he began to snore. However, I quickly noticed the sound of his snoring was different. It came in starts and stops, a choppy noise that wasnt like his normal snoring at all.

In fact, it sounded like he was having a more difficult time breathing.

Concerned, I woke him up and had him take it off. Not only did the Anti Snore Chin Strap fail to stop Johns snoring, it was now keeping me awake with worry it would interfere with his breathing while he slept.

Lifestyle Changes To Treat Sleep Apnea

Multiple lifestyle factors may be able to reduce severity of OSA symptoms.

  • Weight Loss: Research shows that for many people, losing weight can lead to an improvement in OSA symptoms. For this reason, medical professionals might recommend diet and exercise or even bariatric surgery to their obese patients with OSA.
  • Throat Exercises: Also called oropharyngeal exercises, these repeated movements of the tongue, soft palate, and throat have been demonstrated to reduce OSA symptom severity. In several studies using these exercises, participants see improvements in OSA symptoms after doing the exercises regularly for three months. Experts do not yet know why these exercises are effective.
  • Changing Your Sleeping Position: Positional therapy can reduce OSA symptom severity in some people. Back sleeping is associated with more severe symptoms for many people with OSA, so positional therapy generally involves trying to train a person to stop sleeping on their back.
  • Avoiding Alcohol and Certain Medications: People with OSA should avoid alcohol and sedatives in the hours before sleeping, if possible. Alcohol is thought to increase OSA symptoms by relaxing muscles associated with the airway, thereby causing blockages.
  • Quitting Smoking: Smoking cigarettes increases a persons risk of having OSA, so doctors often recommend that smokers with OSA quit smoking. However, more research is needed before we can be certain that quitting smoking helps reduce OSA symptoms.

Continuous Positive Airway Pressure

4 Pack Silicone Clipple Magnetic Anti Snore Stop Snoring ...

Doctors consider continuous positive airway pressure the gold standard treatment for OSA, and it is usually the first treatment offered to people diagnosed with moderate or severe obstructive sleep apnea. Generally, a CPAP machine plugs into an outlet and sits next to a sleepers bed, connecting to their face via a tube leading to a mask that covers either their mouth and nose or just their nostrils. Then, the CPAP machine blows air into the sleepers airway, making it less likely to close as they breathe during sleep.

CPAP is considered the most effective OSA treatment, but many people prescribed CPAP do not use it as often as they should. CPAP therapy is less effective at treating central sleep apnea. Sometimes oxygen is combined with a CPAP machine when treating people with CSA or mixed sleep apnea.

CPAP machines only push out air at one rate, which is often calibrated by the doctor to the average rate needed by the sleeper. Since some people have trouble tolerating this continuous level of air pressure, other options are available.

When You Need More Help

If lifestyle strategies dont help you stop snoring, see your doctor, a sleep specialist, or an otolaryngologist, who may recommend an overnight sleep test to see whether you have OSA.

For primary snoring or mild to moderate OSA, an oral appliance worn during sleep shifts the lower jaw and tongue forward, keeping the airway open.

Over-the-counter devices are available, but our experts recommend one thats customized for you, which is usually covered by insurance. In 2015, the American Academy of Sleep Medicine and American Academy of Dental Sleep Medicine noted that there is moderate evidence that they improve quality of life.

The gold standard for moderate to severe OSA is continuous positive airway pressure, or CPAP, which delivers air to your airway during sleep to keep it open. A 2016 study in The New England Journal of Medicine found that CPAP helped reduce daytime sleepiness and snoring for people who were followed for an average of almost four years.

When other methods fail or are intolerableCPAP adherence may be as low as 50 percentsurgery, such as a procedure to remove excess tissue in the nose, mouth, or throat, may be an option to help you stop snoring.

No 12 Easy To Keep Clean

Consult your dentist and the MAD manufacturers instructions on how to clean your MAD.

I would suggest that broadly speaking you should use a toothbrush and toothpaste on the device before/after cleaning your teeth.

Floss if you have the enthusiasm and discipline and consider using a chlorhexidine mouthwash too.

Having worked in this industry for longer than I care to remember, I know that MADs must be:

  • Easy to keep clean
  • Not absorb cleaning fluid or saliva
  • Ideally not have intricate mechanisms to retain food and bacteria
  • Be entirely made of one material

Bacterial biofilm will form on the device.

Its important to pay extra attention to your oral hygiene .

In : Anti Snoring & Air Purifier

Made of plastic and medical-grade silicone, this anti-snoring gadget comes in a reusable case and is supposed to fit snugly in the wearers nostrils.

The packaging claims the front snore reduction vents have been scientifically designed to maximize airflow through the nasal passage ways and the silicone prongs that are inserted into the wearers nose comfortably fit different size of nasal passages.

Right out of the box, the 2 in 1: Anti Snoring & Air Purifier gave off what John described as a gross plastic chemical smell when he held it up to his nose. In fact, it was so strong he couldnt try it on until we thoroughly washed it with soap and warm water twice to reduce the harsh scent.

Treating Sleep Apnea At Home

You may be able to treat mild sleep apnea with some lifestyle changes. Your doctor may recommend that you:

  • Lose weight. About half of people with sleep apnea are overweight. If you have some extra weight, you might also have extra tissue in your throat that makes it harder to breathe. Losing weight — even a few pounds — can often improve your symptoms.
  • Donât use alcohol and sleeping pills. They decrease the muscle tone in the back of your throat, which can interfere with air flow.
  • Change sleep positions. You may breathe more easily if you stay off your back. Hereâs a trick to keep from rolling over: Put two tennis balls into a tube sock and pin it to the back of your pajamas.
  • Stop smoking. Smoking can increase swelling in your upper airway, which may make both snoring and apnea worse.
  • Treat allergies. Nasal allergies swell the tissues in your airways and make them narrower, so itâs harder to breathe. Ask your doctor how to get them under control.

Expert Opinion: Dr Ronald A Popper The Southern California Sleep Disorders Medical Center

Dr. Ronald A. Popper, The Southern California Sleep Disorders Medical Center

Dr. Ronald A. Popper is board certified in Sleep Disorders Medicine and is considered to be one of the pioneers in sleep medicine. He established The Southern California Pulmonary and Sleep Disorders Medical Center, where he currently holds the position of Medical Director. In addition to his medical practice, Dr. Poppers delivers lectures and publishes articles on obstructive sleep apnea and other sleep disorders.

Chin straps are used in patients who open their mouth while they are asleep, thus allowing the positive airway pressure generated either by CPAP or Provent therapy to escape through their mouth. When this happens, neither therapy will work as designed. The mouth must remain closed when using CPAP or Provent therapy. Traditional commercially available chin straps are made from stretchable velcro. The problem with stretchable velcro is that you can open your mouth even while this is in place because of the elasticity of velcro.

Alter Your Sleep Position

Though a small change, altering your sleep position can reduce sleep apnea symptoms and improve your nights rest. A 2006 study found that more than half of obstructive sleep apnea cases are dependent on position.

have shown sleeping on your back called the supine position can worsen symptoms. For some adults, sleeping on the side can help breathing return to normal.

However, a found that children with sleep apnea sleep better on their backs.

Discuss body positioning and your sleep apnea symptoms with your doctor to evaluate your options for treatment.

Verywell / Sabrina Jiang

Snoring may seem like a laughable habit until you realize the very real effects. Snoring is a serious breathing disorder that harms social interactions and your health. It can be caused by a number of lifestyle and genetic factors, per Mayo Clinic. The anatomy of your mouth and sinuses, alcohol consumption, allergies, a common cold, and your weight can all bring on a case of sawing logs.

Snoring isnt only a nuisance for snorers, it can disrupt their bedtime companions as well. You can DIY snoring solutions, but it is recommended to see a doctor to check for more serious conditions like obstructive sleep apnea first.

Keep in mind that these devices should only be considered if obstructive sleep apnea has been ruled out with a sleep study administered by a healthcare provider.

Here are the best anti-snoring devices on the market.

No 7 Can I Adjust The Position Of My Anti

â?Anti Snoring Nose Clip Silicone Soft Sleep Apnea Aid CPAP ...

The ability to adjust the forward position of your jaw impacts upon both comfort and how well it works.

The optimum position is unique to you. It has to be just far enough forward to prevent you from snoring, yet not so far forward, it hurts your jaw joints.

Fixed devices take an arbitrary approach to the forward position of your lower jaw .

This primitive design is likely to be less than ideal for comfort and of course effect.

Which leads neatly in to how anti-snoring devices work:

How do anti-snoring devices work?

In a word: protrusion.

In the short video clip below, I explain how protrusion opens the space behind your tongue .

The high-end devices using nylon or chrome are thinner and less bulky but are also difficult to adjust. This does not mean to say its a problem, they just have to be perfectly made.

So, if its not then it needs to be remade: Do not accept a poor fitting custom made anti-snoring device.
