Endo 2019 News: Cpap Machine Improves Weight Loss In Dieting Adults With Obesity Sleep Apnea

When trying to lose weight by cutting calories, people who have obstructive sleep apnea in addition to obesity can lose more weight if they treat their sleep disorder with an overnight CPAP, or continuous positive airway pressure, machine. This finding of a new study was at ENDO 2019 in New Orleans, La.

“Some people may think if they lost weight, they may not need the CPAP machine,” says lead investigator Yuanjie Mao, MD, PhD, an endocrinology fellow physician at the University of Arkansas for Medical Sciences , in Little Rock, Ark. “However, our study finds the opposite for people with obesity and OSA who try to lose weight by restricting calories: CPAP treatment can actually result in more weight loss.”

OSA, most often caused by obesity in adults, is a common sleep disturbance in which breathing repeatedly stops and starts during sleep. Wearing a CPAP mask during sleep is the first-line treatment of OSA and is widely recommended with weight loss for patients with obesity, despite a lack of scientific evidence to support this strategy, Mao says.

A total of 89 patients reported no symptoms of OSA and therefore had no OSA treatment. Another 164 patients had OSA symptoms but did not receive a CPAP machine, whereas 47 patients had OSA symptoms and received treatment with a CPAP machine. Because not all the patients had a sleep study, the researchers could not determine their OSA severity, said senior investigator Peter Goulden, MD, FRCP, an endocrinologist at UAMS.

New Research Shows Sleep Apnea Treatment With Cpap Therapy May Help With Weight Loss

A recent study conducted at the University of Arkansas looked at the effects of CPAP sleep apnea treatment on weight loss. The study involved three groups: one non-sleep apnea group, one sleep apnea group who received CPAP, and one sleep apnea group who didn’t receive CPAP. Everyone involved in the study was also obese at the beginning of the study.

Each participant started on a weight loss program to help lose weight. This included restricting calories to 800 per day and an exercise program, as well as weekly counseling and cognitive behavior therapy. The researchers followed their progress over 16 weeks to determine the results. 

The CPAP therapy group lost an average of 5.7 pounds more than any other group. At the end of the study, the CPAP group lost an average of 26.7 pounds. The sleep apnea group who didn’t receive sleep apnea treatment lost an average of 21 pounds and the non sleep apnea group lost an average of 19.

Researchers in the study concluded that CPAP therapy was correlated with better weight loss on the weight loss program. They noted that this could be because CPAP may help improve sleep quality, which may have a major impact on weight management. 

In addition, other studies have found that sleep apnea treatment with CPAP can help reduce belly fat, also known as visceral fat, even without significant weight loss. Visceral fat is a concern because it increases your risk for serious conditions like diabetes and heart disease. 

Cpap Therapy May Still Be Needed To Treat Severe Sleep Apnea Even After Weight Loss

It’s important to remember that extra weight is not the only cause of sleep apnea. A small jaw, a large neck, large tonsils and adenoids, genetic predisposition, and other factors could be contributing to this sleep disorder. In all likelihood, a case of severe sleep apnea is caused by a combination of these factors–so weight loss alone may not completely eliminate the problem. CPAP therapy is the gold-standard therapy for sleep apnea and is the only treatment that is 100% effective when used properly every night .

Another study3 investigated weight loss in patients with severe obstructive sleep apnea. After losing on average 20+ pounds, the subjects’ AHIs decreased by about 25 events per hour! However, the patients’ AHI was still around 30 episodes per hour–which meets the criteria for moderate to severe sleep apnea . 

A similar result was observed when sleep apnea patients underwent weight-loss surgery4. Though the frequency of their apnea events decreased significantly, the mean AHI post-surgery was still consistent with moderately severe OSA.
