Dear Dr. Roach I have a medical condition that currently requires me to take 60 mg of prednisone per day, resulting in insomnia. I get approximately five hours of sleep per night. Sometimes Im able to take a one-hour nap in the afternoon. Is there anything I can do to reduce the insomnia? M.H.S.
Answer Prednisone is a powerful anti-inflammatory and immune-system suppressor that is used for many conditions. It can cause some degree of insomnia in 50 to 70 percent of those who take it, but the higher the dose, the more likely the insomnia.
It sounds like crazy advice, but many of my patients have had luck following my recommendation to take the prednisone immediately before bedtime. Prednisone takes time to work, and some people are able to fall asleep before the effects of the medicine kick in.
Unfortunately, others have medical conditions that require them to take prednisone twice daily. This is more effective, but its more likely to cause side effects, including insomnia.

Coping With The Insomnia
Even when the dose of dexamethasone was reduced, I still had sleep problems and just tried to cope as best I could.
I tried to stay calm, and remind myself not to stress out when I had difficulty sleeping. I did simple breathing exercises to take my mind off any discomfort and help me relax.
I made a concerted effort to eat healthy food, avoid stimulants and heavy food in the evening. And I went out every day for some fresh air and daylight, even if it was just for a short walk in the park.
Essentially, I just tried to manage my insomnia the same way I would do if I wasnt having chemotherapy.
Looking back, I dont think theres a lot more I could have done to improve my sleep other than taking me off the dexamethasone altogether.
I didnt take any sleeping tablets, as I already felt up to the hilt in medication. So largely I just put up with it and tried to remember that I was going through all this to try to cure me of cancer.
And fortunately, Im still all-clear ten years later. July 2021 marks a decade since my last treatment, so its a major milestone.
My advice to anyone experiencing sleep problems during chemotherapy is to talk it through with your oncologist. At the end of the day, theres a reason they use it but they still might be able to help you with your sleep problems or other side effects.

Insomnia From Cortisone Shot

I had a cortisone shot on the 24th of December. It is over two weeks later and I am not sleeping. Some nights zero sleep, some nights and hour here or there, and only one night in the two weeks did I actually get about five hours straight.Anyone have any suggestions on what might help me as the doctors will not give me anything and state that it has nothing to do with the shot. Well, I have not EVER had any trouble sleeping in my 44 years. This started the day I got the shot. I have tried Vicatin, Advil PM, Ambien and Lorazepam . Nothing has worked.Also, anyone who has had this happen to them, how long did the insomia last???? Is there a chronic insomnia treatment?Please help…I can’t live like this..

Stop taking other people’s pills and pills that aren’t meant to address sleep issues. Go see a doctor to work out this problem, whatever the cause. Lest you think I don’t understand, I have had a sleep disorder for 18 years which accompanies my fibromyalgia and have experienced many consecutive sleepless nights and even weeks or months at a time with only 3-4 hours sleep in 24 I know it’s miserable and frustrating. I also know first-hand that doctors aren’t always helpful but they’re a place to start and a better option than ignorantly taking OTC meds or other people’s prescriptions.

How To Cope With Side Effects

- weight gain try to eat well without increasing your portion sizes so you dont gain too much weight. Regular exercise will also help to keep your weight stable.
- indigestion take prednisolone with food to reduce the chances of stomach problems. It may also help if you avoid rich or spicy food while youre taking this medicine. If symptoms carry on, ask your doctor if you may benefit from taking an additional medicine to protect your stomach.
- sleep problems take prednisolone in the morning so the levels are the lowest at bedtime
- restlessness if youre restless when youre trying to sleep, take prednisolone in the morning so the levels are the lowest at bedtime
- sweating a lot try wearing loose clothing, use a strong anti-perspirant and keep cool with a fan. If this doesnt help, talk to your doctor as you may be able to try a different medicine.
